TO ALL WHOM IT MAY CONCERN:

KNOW YE, That John Conley, Private of captain D. Ketchum's company in the twenty fifth regiment of U. States Infantry who was enlisted the twenty first day of March eighteen hundred and fourteen for the period of During the War is hereby honorably discharged from the army of the United States, on the surgeon's certificate, as being disqualified to perform the duties of a soldier by reason of a Wound by the bursting of a Bomb which fractured the bones in his left Foot in the battle with the Enemy at Chippewa.

Said John Conley was born in the Town of Norwalk in the State of Connecticut is about Eighteen years of age, five feet six & ½ inches high, light complexion, black eyes, brown hair and by occupation, when enlisted, a Cordwainer.

Given at Greenbush cantonment,
this Twenty Sixth day of February 1815.
