FORM of CERTIFICATE of the Cashier of the Bank of England, acknowledging the Transfer of Bank Annuities for the Purchase of Life Annuities.
IN pursuance of an Act, passed in the Forty-eighth Year of the Reign of His present Majesty, intitued, An Act [here insert the Title of the Act] I do hereby certify, That the Sum of £3 per Centum Consolidated [or, Reduced] Bank. Annuities hath been this Day transferred to the Commissioners for the Reduction of the National Debt, by or on the Behalf of for the Purchase of a Life Annuity under the Provisions of the said Act.
