2006/05# I think the most important thing should be first in the template — that the image is in the Public Domain and where it comes from.

The template also mentions both USACMH (the U.S. Army Center for Military History) and USAMHI (US Army Military History Institute. I think it should be made clearer how those two relates to each other.

Too long ... proposed simpler version edit

This template is inappropriately long. It makes it sound as if this site were actually the USACMH. The appropriate role of a template is to simply identify the provenance and the copyright status. Disclaimers about copyright status being uncertain are not ok; a document with unknown copyright status isn't even allowed to be posted here in the first place. The template ought to be something along the lines of:

 
This image or document is from the collection of the U.S. Army Center of Military History, most of which was produced by a U.S. Army soldier or employee during the course of the person's official duties. As a Work of the United States Government, it is in the public domain.
