Missing case table

I consolidated the missing cases into one table. I think there are a few issues

• Duplicate content in the source. Bulk.resource.org has multiple copies of some cases (see [1] and [2]). OpenJurist also the same duplicates (see [3] and [4]) because it is just a copy of bulk.resource.org.
• Improper formatting in the source file. See 139 U.S. 377's problems on bulk.resource.org.
• Multiple citations for one opinion. According to public.resource.org, both 139 U.S. 342 and 139 U.S. 377 go to the same case. I'm not sure if that is right. Also, BenchBot may skip the second citation when the case is automatically formatted.
