There is bug in the Wikisource file upload tool, which is not requesting any author or license information. I am copying the information from Wikipedia by hand.
Note that I was asked to copy the PDF file to Wikisource, which I was attempting to do. The original file is here: http://en.wikipedia.org/wiki/Image:BAssoc1859pp200-228.pdf
